committed to protecting the privacy, dignity, and confidentiality of participants,
families, carers, guardians, nominees, and representatives.
We understand that the information we collect while providing disability support
services may be personal and sensitive. We handle all information in accordance
with applicable privacy laws, NDIS Practice Standards, and our organisational
policies.

To provide safe and effective supports, we may collect:
Participant Information

 Date of birth

 Contact details
 Emergency contact information
 NDIS participant number
 Plan details
 Funding arrangements
 Goals and support needs
Health and Support Information
Where relevant to service delivery:
 Medical information
 Medication information
 Allied health recommendations
 Behaviour support information
 Mobility requirements
 Communication needs
 Risk assessments
 Incident records

Representative Information
We may collect information relating to:
 Parents
 Guardians
 Nominees
 Support coordinators
 Plan managers

 Deliver supports safely and effectively

 Meet participant goals
 Manage risks and incidents
 Coordinate with health professionals and support networks
 Meet legal, contractual, and NDIS obligations
 Improve service quality
Consent
Where possible, we obtain consent before collecting, using, or sharing personal
information.
Consent may be provided by:
 The participant
 A parent or guardian
 An authorised representative
 A nominee
Consent may be withdrawn at any time, subject to legal and operational
requirements.
Disclosure of Information
We only disclose information where:

 The participant has consented
 Disclosure is necessary for service delivery
 Required by law
 Required by the NDIS Commission
 Required for health and safety purposes
 Required to prevent serious harm
Information may be shared with:
 Support coordinators
 Plan managers
 Health professionals
 Allied health providers
 Emergency services
 Government agencies where legally required
Storage and Security
Participant records are stored securely through physical and electronic systems.
We take reasonable steps to:
 Restrict access to authorised personnel
 Protect information from unauthorised access
 Maintain confidentiality
 Securely dispose of records when no longer required
Access to Records
Participants may request access to information we hold about them unless legal
restrictions apply.
Requests should be made in writing to CALM by CARM.
Feedback and Complaints
Participants have the right to raise concerns regarding privacy practices without fear
of disadvantage.
